The game's story revolves around Isaac, a young boy who lives with his mother in a small house. Isaac's mother, believing that God has ordered her to sacrifice her son, tries to kill Isaac. However, Isaac escapes to the basement of their house, where he must fight for survival against various monsters, including his own psyche's manifestations.
